- I hereby acknowledge that participation in dance classes and activities involves some
risk of personal injury. I acknowledge and agree to assume and accept the risks
associated with this, and assume full responsibility for my own participation. - By signing this release form, I assume any and all risks related to the use of any and all
spaces utilized by COLLECTIVE HEELING. - I will not hold COLLECTIVE HEELING liable for any personal injury, or any personal
property damage, which may occur before, during, or after classes. - I agree to adhere to the class and facility rules and take any and all responsibility for my
own behavior. - I certify that I have no injury, be it physical, emotional, or mental, condition, or limitation,
that prevents me from participating, or could interfere with my safety during the exercise
activities at COLLECTIVE HEELING. - By signing this release form, I assume all risks in connection with participating with
COLLECTIVE HEELING.
Photo Release
- I authorize COLLECTIVE HEELING to utilize photographs, videos, and
audio of myself for promotional material within social media. - I authorize COLLECTIVE HEELING to store photographs, videos, and audio of myself
for teaching purposes or future promotional purposes.

- I acknowledge the ever changing stress the global pandemic of Covid-19 and its impacts
upon the ever changing British Columbia Provincial Health Guidelines. - I acknowledge that I will follow the most current and up to date Provincial Health Orders
with relation to activity and exercise with COLLECTIVE HEELING. I also recognize that
COLLECTIVE HEELING may put additional safety measures in place beyond those
currently ordered by the Provincial Health Order. - I understand and accept the risk of exposure to Covid-19 while participating with
COLLECTIVE HEELING. - I agree to COLLECTIVE HEELING storing my Covid-19 vaccination information in a
confidential manner, OR I agree to verify my Covid-19 vaccination status at every class
with COLLECTIVE HEELING that I attend.

- For users that register on our website, we store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.
- If you have an account on this site, or have left comments, you can request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.
